This clinical trial is looking at two different surgical methods to treat a condition called temporomandibular joint (TMJ) disc displacement, which can cause pain and difficulty in opening the mouth. The researchers want to find out which method—arthroscopic lysis and lavage or operative arthroscopy—works better in relieving pain, improving the ability to open the mouth, and reducing disability related to this condition. Participants will undergo one of these surgeries while asleep (under general anesthesia) and will not know which treatment they received. They will have follow-up appointments at 1, 3, 6, 12, and 24 months after the surgery to check their recovery and complete surveys about their quality of life.
To join this study, participants must be at least 18 years old and have verified TMJ disc displacement along with pain or disability rated at a certain level. However, people who have had previous TMJ surgery, have certain medical conditions, or cannot provide informed consent will not be eligible. This trial is not yet recruiting, but it aims to gather important information that could help improve treatment options for TMJ disorders in the future.

Eligibility criteria
- Inclusion Criteria:
- • Uni- or bilateral DDwR verified with clinical and magnetic resonance imaging (MRI) findings
- • TMJ pain ≥ 3 (NRS) and/or TMJ disability ≥ 3 (NRS)
- • Age ≥ 18 years
- Exclusion Criteria:
- • Prior open TMJ surgery
- • Patient diagnosed with rheumatologic joint disease
- • ASA (American Society of Anaesthesiologists) \> 3
- • Patient unable to verify informed consent
