This study is testing whether giving lidocaine through an IV during thyroid surgery (with nerve monitoring to protect the recurrent laryngeal nerve) offers any advantages over the usual anesthesia approaches. Participants are randomly assigned to one of three plans during surgery: (1) intermittent fentanyl with sevoflurane, (2) continuous lidocaine infusion with sevoflurane, or (3) continuous remifentanil infusion with sevoflurane. The researchers want to see if lidocaine helps keep the depth of anesthesia steady, reduce movement during surgery, and lower the need for other pain medicines.
Eligible adults are 20 to 75 years old and are having open thyroid surgery with nerve monitoring. Exclusions include allergies to opioids or lidocaine, significant kidney or liver problems, or certain heart or rhythm issues. If you join, you’ll undergo preoperative tests and be assigned to one of the three groups, with ongoing monitoring during and after surgery. They’ll track things like surgeon satisfaction, pain scores in the recovery room, pharmacy use for pain relief in the first 24 hours, blood pressure and heart stability, and common side effects such as sore throat or nausea. The trial is currently enrolling in Kaohsiung, Taiwan, and aims to enroll about 100 adults, with results expected by late 2025.

- Inclusion Criteria:
- • Age 20-75 years old
- • Receiving open thyroid surgery
- • Using intraoperative neurophysiological monitoring
- Exclusion Criteria:
- • Allergic to opioids or lidocaine.
- • Unsuitable for prolonged lidocaine infusion due to impaired cardiac function or severe arrhythmias.
- • Impaired renal function (eGFR \< 30).
- • Liver dysfunction (Child-Pugh score B/C).
