The Kinematic and Mechanical Alignment Randomized Trial is studying two different methods of knee replacement surgery for people with painful knee arthritis. One method, called mechanical alignment, focuses on making the new knee as straight as possible, while the newer technique, known as restricted kinematic alignment, customizes the placement of the new knee to match each person's unique knee shape. Researchers want to find out which method leads to better outcomes, like improved walking and overall knee function. All participants will receive robot-assisted surgery to ensure that the knee implants are placed accurately.
To be eligible for this trial, you need to be at least 18 years old and have knee osteoarthritis that requires a total knee replacement. Participants will be randomly assigned to one of the two surgical techniques and will be asked to walk and complete questionnaires at different times before and after the surgery. This study aims to help surgeons understand which knee replacement method might work best for patients, potentially leading to better recovery and long-term results.

Eligibility criteria
- Inclusion Criteria:
- • Adult aged 18 years or older
- • Knee osteoarthritis requiring total knee arthroplasty
- • Provide informed consent
- Exclusion Criteria:
- • Inability to provide informed consent (e.g. cognitive disability, language barrier)
- • Revision knee surgery
- • Simultaneous bilateral knee surgery
- • Recent lower extremity surgery or trauma interfering with gait assessment
- • A CT scan cannot be obtained prior to surgery
- • Inability or unwillingness to comply with the study protocol
