This clinical trial is looking at how different ways of giving dexamethasone—a medication used to reduce inflammation—affect pain and recovery for patients undergoing total hip replacement surgery. Specifically, it will compare giving the medication through an intravenous (IV) line versus directly around the nerves in the hip area. The main focus is on measuring pain levels after surgery, along with other factors like inflammation and how satisfied patients feel with their care.
To participate in this study, patients need to be between 65 and 100 years old and scheduled for hip replacement surgery. They should be able to understand the study and share their experiences with the research team. However, individuals with certain conditions, such as infections at the surgery site or serious medical issues, won't be eligible. Those who join the trial can expect to help improve future recovery methods for hip surgery, as researchers aim to find the best ways to manage pain and improve healing after surgery.

Eligibility criteria
- Inclusion Criteria:
- • patients scheduled for total hip arthroplasty
- • patients aged \>65 and \<100 years
- • patients able to provide informed consent
- • patients able to reliably report symptoms to the research team
- Exclusion Criteria:
- • inability to provide first-party consent due to cognitive impairment or a language barrier
- • infection at the site of the regional block,
- • coagulation disorders,
- • immunodeficiency,
- • American Society of Anesthesiologists (ASA) physical status of IV or higher,
- • history of regular steroid medication.
