This clinical trial is studying the effectiveness of two different medications, dexmedetomidine and fentanyl, when added to a specific type of pain relief technique called an erector spinae plane block for patients who are having a simple nephrectomy, which is a surgery to remove a kidney. The main goal is to see which medication helps patients wait longer before they need to ask for more pain relief after their surgery. Researchers will also look at how much pain patients feel at different times after surgery and how much additional medication they need in the first 24 hours.

To be eligible for this trial, participants should be adults over 18 years old who are generally healthy according to a specific health classification. However, patients who have certain conditions, such as a very high body weight, infections at the injection site, or a history of drug allergies, will not be included. If someone joins the trial, they can expect to receive one of the two medications during their procedure and will be monitored closely for any side effects or complications related to the pain relief technique. This is important research that hopes to improve pain management after kidney surgery.

Eligibility criteria

  • Inclusion Criteria:
  • Age \>18 years old Both sex patients who are American society of Anesthesiologists class I or
  • Exclusion Criteria:
  • Patient's refusal.
  • body mass index (BMI) ≥40 kg/m2.
  • Local infection at site of block.
  • Spine deformities
  • History of hypersensitivity to the drugs being evaluated
  • Inability to comprehend postoperatively the pain assessment scale/neuropsychiatric disorders.
  • Chronic use of opioids and opioid addiction
