This clinical trial is studying how moderate-to-high intensity aerobic exercise might help women who experience painful menstrual cramps, known as primary dysmenorrhea. The goal is to see if regular aerobic exercise can reduce the severity of these cramps and improve overall quality of life for participants. Women aged 18 to 26 who lead a sedentary lifestyle and experience moderate to severe menstrual pain may be eligible to participate.
Participants will be divided into two groups: one group will engage in a supervised aerobic exercise program tailored to their fitness level, while the other group will continue with their usual care without any extra interventions. Before starting, all participants will fill out a questionnaire about their health and menstrual cycle, which will help researchers understand their conditions better. The study will take place in Madrid, Spain, from February to April 2025, and will be designed to ensure fair and unbiased results. This research could provide valuable information for managing menstrual pain through lifestyle changes instead of medication.

Eligibility criteria
- • Inclusion Criteria
- • Age between 18 and 26 years.
- • Sedentary lifestyle.
- • Body Mass Index (BMI) between 18.5 and 26 kg/m².
- • Regular menstrual cycle (24-38 days).
- • Menstrual pain intensity ≥ 4 mm on the Visual Analog Scale (VAS).
- • Presence of anxiety or stress.
- • Exclusion Criteria
- • Pregnancy or breastfeeding within the last 3 months.
- • Amenorrhea.
- • Use of oral contraceptives in the last 3 months.
- • Presence of an intrauterine device (IUD).
- • Regular alcohol consumption.
- • Active smoking.
- • Chronic endocrine, digestive, neurological, or uncontrolled cardiovascular diseases, and/or psychiatric disorders.
- • Any medical diagnosis contraindicating physical exercise.
