This clinical trial is looking at how different positions during laparoscopic surgery affect pressure in the body and blood flow to the brain. Specifically, the study compares two positions—Trendelenburg (where the patient is tilted with their head down) and Reverse Trendelenburg (where the head is elevated)—along with different types of mechanical ventilation methods. The goal is to find out if these positions and ventilation methods impact the pressure in the esophagus, airways, and brain, and how they affect breathing and blood flow.

To participate in this trial, individuals must be between certain age ranges and be scheduled for laparoscopic surgery that requires either the Trendelenburg or Reverse Trendelenburg position. They should not have significant heart or lung diseases, brain conditions, or other serious health issues that could affect their ability to understand the study. Participants will be randomly assigned to one of the ventilation methods and will have their pressures and blood flow measured at different times during the surgery. This research aims to ensure that patients experience the least amount of physical disturbance during their procedure.

Eligibility criteria

  • Inclusion Criteria:
  • Age: Unrestricted
  • American Society of Anesthesiologists (ASA) Physical Status: Class I-III
  • Surgical Position: Requires the use of either the Trendelenburg position or the Reverse Trendelenburg position during the procedure.
  • Ventilation: Planned mechanical ventilation is required during the procedure, with suitability for one of the following modes: pressure-regulated volume-controlled ventilation (PRVC), conventional volume-controlled ventilation (C-VCV), or pressure-controlled ventilation (PCV).
  • Exclusion Criteria:
  • * Significant Cardiopulmonary Disease:
  • Intracranial Disease
  • Severe Coagulopathy or Current Anticoagulant Therapy
  • Presence of Psychiatric Illness, Cognitive Impairment, or other condition that precludes the ability to understand or comply with the study.
