This clinical trial is studying how certain medical tests, called biomarkers, can help predict how well a person will recover after having a stroke. Researchers want to understand how these biomarkers relate to a patient's ability to move and their overall disability after a stroke. By gathering and analyzing this information, the team hopes to create a tool that can give a reliable prediction of recovery outcomes for individuals who have experienced a stroke.
To be eligible for the study, participants must have had their first stroke in a specific area of the brain called the middle cerebral artery, and it should be between 7 to 30 days since the stroke occurred. They should also have some weakness in their arm or hand. Both men and women over the age of 18 can participate, as long as they or their caregivers can provide informed consent. Participants in the study can expect to undergo various tests, including specialized imaging and brain activity assessments, to help researchers gather the necessary data. It's important to note that individuals with certain medical histories, like previous strokes or other neurological conditions, may not qualify for this study.

Eligibility criteria
- Inclusion Criteria:
- • First stroke in the middle cerebral artery territory.
- • Ischemic or hemorrhagic stroke (7-30 days post-acute event).
- • Upper limb plegia or paresis.
- • Both genders, aged \>18 years.
- • Ability to provide informed consent by the patient or caregiver/responsible relative.
- Exclusion Criteria:
- • Previous stroke.
- • Stroke in territories other than the middle cerebral artery.
- • Inability to provide informed consent.
- • Contraindications to TMS or MRI.
- • History of cancer in the past two years.
- • Presence of other neurological conditions interfering with biomarkers
