Complete Closure After Endoscopic Mucosal Resection of Large Non-Pedunculated Colorectal Polyps

This clinical trial is studying the best way to treat large flat polyps in the colon, which can potentially lead to colorectal cancer. Researchers want to find out if closing the area where the polyp was removed (called defect closure) leads to fewer complications and a lower chance of the polyp returning compared to not closing it. Participants will be divided into two groups: one group will have the area closed after the polyp is removed, while the other group will not have it closed. After the procedure, patients will be checked for any health issues and will have follow-up colonoscopies six months and 18 months later.

To be eligible for this trial, participants must be adults 18 years or older who are scheduled to have the polyp removal procedure. Some individuals are not eligible, including those with certain health conditions, like inflammatory bowel disease, or those who are pregnant. If you decide to join the study, you will help researchers learn more about the safest and most effective ways to treat these large polyps, which is important for preventing colorectal cancer.

Eligibility criteria

  • Inclusion Criteria:
  • adult ≥18 years old
  • patients undergoing EMR for a large (≥20mm) colorectal LSL
  • patients providing written and informed consent for study participation.
  • Exclusion Criteria:
  • inflammatory bowel disease;
  • non-elective colonoscopy;
  • poor general health (American Society of Anesthesiologists classification \>III);
  • coagulopathy or thrombocytopenia (international normalized ratio ≥1.5 or platelets \<50 x 109/L);
  • pedunculated polyps (Paris class Ip, Isp);
  • overt signs of deep submucosal invasive cancer (JNET 3);
  • appendiceal orifice or terminal ileum invasion;
  • pregnancy.
