This clinical trial is investigating how to better diagnose and treat urinary tract infections (UTIs) in adults. Specifically, it compares two methods: the traditional urine culture test and a newer DNA test called polymerase chain reaction (PCR). If you have symptoms of a UTI and are 18 years or older, you may be eligible to participate. To qualify, you need to be presenting with UTI symptoms and have Medicare or Medicaid insurance. However, if you have certain medical conditions, like a chronic urinary catheter or have had a UTI or antibiotics in the last 21 days, you won't be able to join.

If you decide to participate, you will be randomly assigned to one of the two testing groups. You'll fill out a couple of questionnaires at the start and again after finishing any prescribed antibiotics. The study lasts up to 21 days, and you’ll be contacted the day after your antibiotics are scheduled to end to complete the questionnaires. This trial aims to find out which method is more effective for diagnosing and treating UTIs, which could lead to better care for patients in the future.

Eligibility criteria

  • Inclusion Criteria:
  • Adult men and women 18 years and older
  • Presenting to clinic with symptoms of UTI or cystitis
  • Patients with Medicare and Medicaid insurance
  • Exclusion Criteria:
  • Urinary diversion of any type
  • Chronic indwelling urinary catheter
  • Diagnosed UTI within the previous 21 days
  • Antibiotic therapy within the previous 21 days
  • Use of phenazopyridine within the previous 21 days
  • Commercial insurance
  • Non-English speaking
  • Pregnant patients
