This clinical trial is investigating different ways to help people with resistant hypertension, which is high blood pressure that doesn’t respond well to treatment. Over the course of 12 months, the study will compare three groups: one group will use a self-management program supported by an AI chatbot, another group will manage their blood pressure without the chatbot, and the third group will receive standard care from their healthcare provider. The main goal is to see how these different approaches affect blood pressure levels and how well participants stick to their medication plans after 6 and 12 months.
To participate, individuals must be at least 18 years old and currently taking three or more different blood pressure medications, including a diuretic, with a starting blood pressure above 130/80 mmHg, or four medications regardless of their blood pressure. However, people who have difficulty monitoring their blood pressure due to cognitive issues, struggle with taking their medications regularly, or have certain health conditions, such as being pregnant or having a serious illness, would not be eligible. Participants can expect to engage with a self-management program that aims to empower them to take control of their blood pressure, with support systems in place, depending on the group they are assigned to.

Eligibility criteria
- Inclusion Criteria:
- • 1. ≥18 years of age
- • 2. receiving ≥3 antihypertensive medications of different classes, including a diuretic, and baseline BP \>130/80 mmHg; or receiving ≥ 4 medications regardless of BP
- Exclusion Criteria:
- • 1. unable to conduct self-monitor BP because of cognitive dysfunction
- • 2. poor adherence to medication
- • 3. poor digital capabilities
- • 4. pregnant
- • 5. terminal disease
- • 6. an acute cardiovascular event in the previous 3 months
