This clinical trial is studying a new way to give high-flow oxygen therapy to patients who are having trouble breathing due to a condition called hypoxemic respiratory distress (HRD). The goal is to see if using a special camera to measure how much air is in the lungs can help doctors adjust the oxygen flow better, potentially reducing the need for patients to be put on a breathing machine (intubation). Participants will be either treated with standard high-flow oxygen therapy or with the new method that uses lung volume measurements to guide treatment.
To be eligible for this trial, patients must have certain signs of HRD, such as breathing rapidly and having low oxygen levels in their blood. They should not have a history of chronic breathing problems or specific urgent medical conditions. The trial is not yet recruiting participants, but if someone is eligible, they can expect to receive either the standard treatment or the new method during their time in intensive care. This trial aims to improve care for patients struggling with breathing issues in a hospital setting.

Eligibility criteria
- Inclusion Criteria:
- * Patients with hypoxemic respiratory distress define by the following 4 criteria:
- • Respiratory rate ≥ 25 breaths per minutes
- • A ratio of the partial pressure of arterial oxygen (PaO2) to the FiO2 ≤ 300 mmHg while the patient is breathing oxygen at a flow rate of 10 liters per minutes or more
- • A partial pressure of arterial carbon dioxide (PaCO2) not higher than 45 mmHg
- • An absence of clinical history of underlying chronic respiratory failure
- Exclusion Criteria:
- • A do-not-intubate order
- • Pregnant or lactating woman
- • Cardiogenic pulmonary edema
- • Exacerbation of asthma
- • Hemodynamic instability with use of vasopressor at significant dose (norepinephrine \> 0.5 mg/h)
- • A Glasgow Coma Scale of 12 points of less
- • Contraindications to non-invasive ventilation
- • Urgent need for endotracheal intubation
- • High flow oxygen therapy started within 48 hours before inclusion
- • Patient under legal protection or deprived of liberty
