This clinical trial is studying a new digital tool called eDoseCKD, designed to help community pharmacists in Nova Scotia safely manage medications for people with chronic kidney disease (CKD). The goal is to ensure that patients with CKD, especially those at stages 4 and 5, receive the right medication doses to protect their kidney health and avoid harmful side effects. The trial will look at how well this tool works in real-life pharmacy settings and whether it improves the quality of care for patients.
To participate in this trial, individuals need to be between the ages of 65 and 74, have a specific level of kidney function (eGFR less than 60), and be taking at least one medication that the tool targets. Unfortunately, people on dialysis or those who cannot give informed consent will not be eligible. Participants can expect to be surveyed about their care experience, and pharmacists will also share their thoughts on using the tool after six months. This study aims to make medication management safer and more effective for those living with chronic kidney disease.

- Inclusion Criteria:
- • individuals with an eGFR \< 60 mL/min/1.73m2 AND
- • receiving at least 1 tool target medication
- Exclusion Criteria:
- • individuals on dialysis
- • unable to provide informed consent
