A Three-arm Randomized Controlled Trial Comparing Transmucosal Augmentation Techniques in Immediate Implant Placement

This clinical trial is designed to study different techniques for placing dental implants right after a tooth is removed, particularly in the front part of the mouth. The goal is to find out which method best helps maintain the bone and soft tissue around the implant, preventing loss as the area heals. To do this, 54 participants will be randomly divided into three groups, each receiving a different treatment approach. Before the surgery, participants will undergo a special type of imaging to ensure they have enough bone for the implant.
To participate in this trial, you should be at least 20 years old, maintain good oral hygiene, and have a tooth that needs to be extracted along with enough bone support for the implant. If you are pregnant, smoke, or have certain health conditions, you may not be eligible. Participants will have their progress monitored over the course of a year, with follow-up scans to assess how well the treatment worked. This study aims to improve dental implant procedures, making them more effective and beneficial for patients needing tooth replacements.

Eligibility criteria
- Inclusion Criteria:
- • At least 20 years old
- • Good oral hygiene defined as full-mouth plaque score ≤ 25% (O'Leary et al. 1972)
- • Presence of an incisor, cuspid or premolar in the maxilla that needs to be extracted for any reason with at least one neighboring tooth present
- • At least 3 mm bone available at the apical or palatal aspect of the alveolus as assessed on CBCT to ensure primary implant stability
- • Intact buccal bone wall (type I) or limited buccal bone dehiscence (type IIa or IIb) at the time of extraction
- • Written informed consent.
- Exclusion Criteria:
- • Pregnancy
- • Systemic diseases
- • Smoking; suppuration
- • \> 1 mm gingival asymmetry between the failing and contralateral tooth
- • Untreated periodontal disease
- • Untreated caries lesions.
