The Digi-STEPSTONES trial is studying a new way to help teenagers with chronic conditions, like asthma or those who have received an organ transplant, as they transition from pediatric to adult healthcare. This program uses digital tools to support these young people and their families, making it easier for them to access care and stay connected with healthcare providers. Their parents will also be asked to fill out a questionnaire to provide additional insights. Participants can expect to be part of a supportive program that aims to make their healthcare transition smoother, while also helping researchers understand the best ways to deliver this type of care digitally. This project not only aims to improve individual experiences but also hopes to provide important information that could benefit other young people with chronic conditions in the future.

Eligibility criteria
- Participants:
- • Arm A - Literate and Swedish-speaking adolescents with asthma/allergy or went through a solid organ transplantation aged minimum 15 years. Their parents will be asked to fill out one questionnaire (proxy).
- Arm B:
