The MNEED trial is a study designed to improve the way critically ill patients receive nutrition while they are in the Intensive Care Unit (ICU). The goal is to develop a better feeding plan that can help prevent feeding intolerance, which is when patients cannot handle the nutrition they are given. This study will include around 2,250 patients from 90 hospitals across the country. To participate, patients must be 18 years or older, unable to eat by mouth, and expected to stay in the ICU for more than 48 hours.
If you or a loved one joins this trial, you will receive care from experienced doctors who will monitor your condition closely. The study will not introduce any new treatments that could increase risks, and all care will follow standard procedures. Participation is completely voluntary, meaning you can choose to leave the study at any time. Your privacy will be protected throughout the trial, and your personal medical information will be kept confidential. If you have any questions or concerns, your healthcare team is there to help you understand more about the study.

Eligibility criteria
- Inclusion Criteria:
- • Age 18 or older Failure of one or more organ systems within 24 hours of admission to the ICU (SOFA score ≥2) Expected to stay in ICU for more than 48 hours Inability to eat orally No EN taboo
- ICU Selection Criteria:
- • ICU in Grade 2 or above hospitals ICU with the ability to carry out intensive care and monitor patient FI ICU type: emergency, medical, surgical, neurosurgical, or comprehensive ICU
- Exclusion Criteria:
- • Patients who have received EN treatment in the past three days AGI Level IV patients Receiving palliative care and expected to die within 48 hours Pregnancy Long-term use of steroids or other immunosuppressants Patients undergoing radiotherapy or chemotherapy for malignant diseases Participation in other clinical studies

- • ICU that refuses to participate in the study ICU that has not passed the ethical review Pediatric ICU
