This clinical trial, called "Automatic Feedback Indicator to Enhance Hospital Discharge Communication," is investigating whether providing hospital teams with ongoing feedback can improve the quality of discharge letters. These letters are important because they summarize a patient's hospital stay and help ensure that both the patient and their primary care doctor have the necessary information for continued care after leaving the hospital. The study will take place at Grenoble Alpes University Hospital and will include around 40 hospital services. Participants will be divided into two groups: one that receives monthly performance updates and another that does not, to see if the feedback helps improve the quality of these discharge letters.
To be eligible for the trial, patients must have been hospitalized for at least 24 hours and must be discharged alive from participating hospital services. Patients who stayed for less than 24 hours, those who passed away during their hospital stay, or those in certain specialized services will not be included. Throughout the trial, data will be collected anonymously, and the goal is to see if the feedback improves the completeness and clarity of discharge letters, patient satisfaction, and reduces the number of patients who have to return to the hospital within 30 days. This research could help improve communication in hospitals across France, making it safer and more effective for patients transitioning from hospital care to their regular doctors.

- Inclusion Criteria:
- • Patients hospitalized for at least 24 hours in participating services.
- • Patients discharged alive directly from participating services.
- Exclusion Criteria:
- • Patients hospitalized for less than 24 hours.
- • Patients who died during hospitalization.
- • Stays in services not meeting inclusion criteria (e.g., psychiatry, long-term care, emergency services with rare direct discharges, or critical care units).
