This clinical trial is studying how changing a patient's position during surgery affects their heart function and blood flow in people who are undergoing a specific weight-loss surgery called laparoscopic sleeve gastrectomy. The researchers want to understand if different positions during the surgery can influence the heart's electrical activity and overall health, especially in patients who are severely obese. This is important because obesity can change how the heart works and may increase the risk of heart problems during surgery.
To be part of this study, participants must be adults aged 18 and older with a Body Mass Index (BMI) of 40 or higher, indicating severe obesity. They should be scheduled for their first laparoscopic sleeve gastrectomy and not have any severe heart issues or other significant health problems that could complicate the surgery. Those who join the study can expect to have their heart activity monitored closely during the procedure to help researchers gather information that could improve surgical safety for future patients.

Eligibility criteria
- Inclusion criteria:
- • Adult patient planned for elective primary laparoscopic sleeve gastrectomy surgery
- • Body Mass Index (BMI) ≥ 40 kg/ / m²
- • Age ≥ 18
- • ASA physical health class II-III.
- Exclusion criteria:
- • Patient refusal to participate in the study
- • Those who underwent revision laparoscopic sleeve gastrectomy
- • Emergency laparoscopic sleeve gastrectomy surgery (stump leakage, etc.)
- • Secondary surgery in addition to elective laparoscopic sleeve gastrectomy
- • Patients with previous recurrent abdominal surgery
- • Patients with electrolyte imbalance
- • Direct laryngoscopy in ramp position
- • Multiple intubation attempts due to difficult intubation
- • Preoperative arrhythmia and heart failure (ejection fraction \< 30%)
- • Renal and liver failure
- • Patients who require \> 8 ml/kg for tidal volume
