This clinical trial is studying a new method to help manage pain after total knee surgery, known as total knee arthroplasty. Researchers want to see how well a combination of two treatments—an injection of a medication called dexmedetomidine into the spinal area and a nerve block in the thigh—can provide relief from pain after the surgery. The goal is to improve comfort and recovery for patients.

To join the study, participants need to be between 18 and 75 years old, in generally good health, and scheduled for knee surgery that will use spinal anesthesia. However, those with certain conditions, like allergies to the medications being tested, neurological issues, or pregnancy, will not be eligible. If you decide to participate, you can expect close monitoring and support from the medical team throughout the process, as they assess how well these pain management techniques work together.

Eligibility criteria

  • Inclusion Criteria:
  • Age from 18 to 75 years.
  • Both sexes.
  • American Society of Anesthesiology (ASA) physical status I-II.
  • Scheduled for total knee arthroplasty under spinal anesthesia.
  • Exclusion Criteria:
  • Patients with neurological or intellectual disability.
  • History of allergy to local anesthetics or dexmedetomidine .
  • Infection at the injection site.
  • Prolonged use of analgesic or sedative medications.
  • Coagulation abnormalities.
  • Pregnancy.
  • Obesity \[body mass index (BMI) \> 30 kg/m2\].
