ClinConnect Summary
The Connect-Home trial is researching a program designed to help patients with Alzheimer's disease and related dementias, along with their caregivers, during the transition from skilled nursing facilities (SNFs) back to home or other living arrangements. The goal is to ease the stress on caregivers and improve the mental and emotional well-being of patients during this important time. The study is currently recruiting participants aged 65 and older who can speak English and have a diagnosis of dementia. Eligible patients must also have a caregiver who is willing to join in the study.
If you or a loved one participates, you can expect support through the transition process, which may help reduce both caregiver strain and challenging symptoms faced by patients. To qualify, patients should not have any planned hospital visits within 30 days of enrolling in the study, and caregivers need to be actively involved and able to communicate effectively. This trial aims to make a meaningful difference for families navigating the complexities of Alzheimer's care.
