The Frail@Home trial is studying how housing counseling can help older adults who have just been discharged from the hospital to stay safe and independent in their own homes. After a hospital stay, many elderly patients, especially those with frailty, may struggle to return to their previous living conditions, especially if their homes are not well-suited for their needs. This trial will compare two groups of participants: one group will receive personalized housing advice, while the other will receive standard care along with an information booklet. The goal is to see if the housing counseling can help older patients spend more days at home and avoid problems like falls or needing to return to the hospital.
To be eligible for the trial, participants must be 70 years or older, have been living at home before their hospital stay, and have some health challenges that affect their daily activities. They should also be planning to return home after discharge. However, individuals with severe memory problems, those in the final stages of life, or those being sent to a nursing home will not be included. If you or a loved one qualifies, participating in this trial could provide valuable support during a challenging transition back home.

Eligibility criteria
- Inclusion criteria:
- • - Patients aged 70 and over who have been living at home up to now, with typical geriatric multimorbidity and a Barthel Index \< 70, who have been admitted to the Wolgast Centre for Geriatric Medicine and whose discharge destination is their own home again. The latter must be in the catchment area of the Wolgast Hospital.
- Exclusion criteria:
- • Persons who are not able to understand the purpose of the study or who are not able to conduct business (e.g. severe dementia, delirium)
- • Patients in a palliative state in the terminal stage
- • Patients who are being discharged to a nursing home
