The Rhode Island - Statewide Postpartum Hypertension Remote Surveillance trial is studying how effective a new care model, called RI-SPHERES, is for women who have high blood pressure after giving birth. This model uses technology to help manage their care and is being compared to a standard program where patients measure their own blood pressure. The goal is to see if the new method is just as good at preventing ongoing high blood pressure and ensuring patients receive the necessary care within a year after delivery.
To participate, women must be at least 18 years old, have recently given birth at a hospital in Rhode Island, and have been diagnosed with high blood pressure during pregnancy or after delivery. They should also speak English, Spanish, Portuguese, or Haitian Creole and own a smartphone. Participants will help researchers learn about the best ways to manage postpartum hypertension, and they will receive support to monitor their health during the study. This trial is not yet recruiting, but it aims to improve care for new mothers facing high blood pressure challenges.

- Inclusion Criteria:
- • Postpartum patient aged 18 years or more at any birthing hospital in Rhode Island
- • Diagnosed during pregnancy or after delivery with hypertensive disorder of pregnant
- • English-, Spanish-, Portuguese-, or Haitian-Creole-speaking
- • Smartphone ownership
- Exclusion Criteria:
- • Prior enrollment in this trial
- • Prisoners or incarcerated people
- • Inability or unwillingness to provide informed consent
- • Inability to communicate with study team, despite an interpreter
