This clinical trial is focused on understanding and improving the way Native Hawaiians manage kidney disease, particularly those who are at higher risk. Native Hawaiians face significant challenges with kidney disease, being much more likely to need dialysis or a kidney transplant compared to other groups. The goal of the study is to create a better support system that respects and incorporates the cultural needs of Native Hawaiians, helping them receive the right care at the right time.
To participate in this study, individuals need to be at least 18 years old, self-identify as Native Hawaiian, and have early-stage kidney disease, which is indicated by specific test results. Participants will also need to have been involved in a related health program recently and be comfortable reading, writing, and speaking English. The trial is currently not recruiting participants, but once it starts, those who join can expect to contribute to important research that aims to create effective and culturally relevant care strategies for kidney disease in Native Hawaiians.

Eligibility criteria
- AIM 1 and AIM 2: Inclusion Criteria: Native Hawaiian (NH) participants from the NKFH KEDS program are:
- • greater or equal to 18 years
- • self-reported as Native Hawaiian
- • eGFR less than 60 ml/min/1.73m2 (Stage 3a-3b CKD) and/or greater or equal to 30 mg/g UACR
- • have participated in the NKFH KEDS Program within 3 months
- • able to read, write, and speak English.
- AIM 1 and AIM 2: Inclusion Criteria: NKFH and PCP staff are :
- • able to read, write, and speak English,
- • greater or equal to 18 years.
