This trial is looking at a new transportation service called door-through-door (D2D) rideshare, which includes a trained companion driver, to see if it helps older adults, especially those with Alzheimer’s disease or related conditions, keep their medical appointments. Participants will be randomly assigned to either the D2D service or a more typical curb-to-curb (C2C) service, which drops them off at the curb. The main question is whether the D2D service leads to fewer missed appointments compared to the C2C service.
To be eligible for this study, participants need to be 60 years or older, living in the community, and able to agree to use the transportation service. They should also have scheduled medical appointments and may have reported memory issues or have a diagnosis of mild cognitive impairment or Alzheimer’s disease. Participants will have the chance to use the D2D rideshare service, which aims to make getting to appointments easier and more reliable. It's important to note that individuals currently using another transportation service or those without stable housing cannot participate. This trial is currently recruiting, and everyone involved will help us understand how this new kind of transportation can support those with cognitive challenges.

Eligibility criteria
- Inclusion Criteria:
- • All participants (or their surrogate) have the capacity to agree to allow Onward service for transportation;
- • have scheduled medical appointments at partner practices of Onward;
- • ages 60+
- • community-dwelling;
- • 60 participants who self-reported cognitive problems, such as memory issue, or have mild cognitive impairment, subjective cognitive decline, or AD/ADRD diagnosis
- Exclusion Criteria:
- • current enrollment in another transportation study;
- • already using any type of transportation service;
- • enrollees with comfort care only;
- • Not wheelchair bound, e.g. if using a wheelchair must be foldable and must be able to transfer
- • Transient, lacking consistent long term housing
