This clinical trial is exploring a new type of weight-loss surgery called Single Anastomosis Sleeve Jejunal Bypass (SASJ) and comparing it to another procedure known as One Anastomosis Gastric Bypass (OAGB). The main goal is to see which surgery is more effective in helping people lose weight, how long the surgeries take, any complications that might arise, and how they affect related health issues over two years after the operation.
To participate in this study, individuals should be between the ages of 18 and 65, have a body mass index (BMI) over 35 with health problems, or a BMI over 40, and be willing to follow the study's guidelines. It's important for participants to be committed to making lifestyle changes and to be able to attend follow-up appointments. However, those with certain conditions, such as severe mental health issues or eating disorders, or who have had previous stomach surgeries, may not be eligible. If you or someone you know is considering weight-loss surgery and meets these criteria, this trial could provide valuable information about these two surgical options.

Eligibility criteria
- Inclusion Criteria:
- • 1. body mass index of over 35 plus the comorbidity presence.
- • 2. body mass index of over 40 with/without comorbidity.
- • 3. patients' willingness to participate in the study.
- • 4. patients' cooperation in follow-up researches.
- • 5. lacking any psychiatric disease.
- Exclusion Criteria:
- • 1. Patients negating to change their lifestyle.
- • 2. drug abuse and/or addiction.
- • 3. eating disorder background (e.g., bulimia nervosa).
- • 4. Patients with contraindications to laparoscopic surgery and (or) unfit for surgery.
- • 5. pregnant patients.
- • 6. previous upper abdominal procedures,
- • 7. reflux symptoms
- • 8. major unstable psychiatric illness.
