This clinical trial is studying a method called the Erector Spinae Plane (ESP) block, which is used to manage pain in patients undergoing liver transplants. The main goal is to find out if the ESP block is safe and effective for helping control pain during and after surgery. Researchers will compare patients who receive the ESP block with those who have the liver transplant without any regional anesthesia techniques to see if there are differences in pain management and side effects.
To participate in this study, individuals must be adults over 18 years old with liver issues like cirrhosis or chronic liver disease. They should also have certain blood test results indicating their liver function is impaired. However, those with very low platelet counts or specific health conditions may not be eligible. Participants will receive the ESP block on the day of their transplant, which involves placing a small tube (catheter) to provide ongoing pain relief. This study is currently recruiting participants, and those who join will help researchers learn more about improving pain management for liver transplant patients.

Eligibility criteria
- Inclusion Criteria:
- • impaired liver function - cirrhosis or chronic liver disease with the development of hypocoagulation (APTT \> 35 sec, INR \> 1.5), thrombocytopenia, patients requiring major abdominal surgery, age \> 18 years, written consent to participate in the study was obtained
- Exclusion Criteria:
- • Platelet count \<50x109, failed ESP catheterization unilateral or bilateral, failed epidural anesthesia attempt, history of allergy or hypersensitivity to local anesthetics, hepatopulmonary syndrome, patients on mechanical ventilation before surgery, patients on CRRT before surgery, refusal to participate in the study
