ClinConnect Summary
This clinical trial, called the DECLARE program, is designed to help caregivers who provide support to patients with dementia. The main goal is to see if the DECLARE program can better support these caregivers by allowing them to complete a self-assessment and increasing their access to social work services. Researchers want to find out if caregivers will choose to complete the assessment, if health professionals find the assessment helpful, and if more caregivers will connect with social workers for assistance.
To participate in this trial, caregivers must be the primary person looking after a home health patient who has been diagnosed with dementia and is 65 years or older. They will need to answer some questions about their caregiving experiences at the start and end of the home health episode and will receive a visit from a trained social worker. This program is not yet recruiting participants, but it aims to provide valuable support to caregivers and improve their confidence in managing the challenges of caregiving.
