This clinical trial is studying a new digital health platform called the BRILLIANT platform, which aims to improve the rehabilitation process for people recovering from conditions like acquired brain injuries, strokes, and cardiovascular diseases. The goal is to make it easier for patients to receive coordinated care and to ensure they have access to effective rehabilitation practices. The trial will involve healthcare programs across four regions in Quebec and will include patients, caregivers, and healthcare professionals who work with these patients.

To participate, patients and caregivers must be able to give consent, and they should speak and read either English or French. Participants can expect to engage with the BRILLIANT platform, which will provide tools for better communication, shared care plans, and support for managing their health. The trial will also measure how effective the platform is by looking at how much rehabilitation patients receive, their quality of life, and their overall experience with care.   • Inclusion Criteria:
  • Clinicians, coordinators, managers, working in the 8 participating programs.
  • Patients and caregivers must be able to:
  • Provide informed consent to collect secondary outcomes (not needed for primary outcome, as identified administrative data will be provided by health regions)
  • Speak and read English or French.
