ClinConnect Summary
This clinical trial, titled "Re-Engineered Discharge for Diabetes Care Transitions," aims to help patients with type 2 diabetes who are hospitalized. The study will test a new approach that includes screening for social needs—like access to food, transportation, or housing—at the bedside and providing support after they leave the hospital. The goal is to see if this method can lower the chances of these patients needing to return to the hospital compared to regular care.
To participate, individuals must be at least 18 years old, have a diagnosis of type 2 diabetes, and be currently hospitalized at a University of Massachusetts-affiliated hospital. They should also have identified at least one unmet social need through a screening survey. However, those who are pregnant, being discharged to a nursing facility, or unable to give informed consent will not be eligible. Participants can expect to receive tailored support aimed at addressing their social needs during their hospital stay and after they go home, helping them manage their diabetes more effectively.
