This clinical trial is studying how to better care for older patients, specifically those who are living with frailty, when they visit the Emergency Department (ED). By identifying frailty as soon as possible during their visit, healthcare teams can tailor their approach to meet each patient's needs more effectively.
To be eligible for this study, participants must be 65 years or older and have their frailty assessed using the Clinical Frailty Scale, which is a tool to measure how much support a person needs. Those who are younger than 65, not assessed for frailty, or choose not to participate will not be included. Participants can expect to receive personalized care based on their frailty level, which may lead to quicker assessments by doctors and potentially shorter stays in the ED. The study will also look at other important outcomes, such as hospital admission rates and health status 90 days after their visit.

- Inclusion Criteria:
- • 65 years of age or older
- • Assessed by CFS with the score being documented in the electronic journal
- Exclusion Criteria:
- • Under the age of 65 years of age
- • Not assessed by CFS
- • Refusal of participation in the study
