ClinConnect Summary

This clinical trial is examining how different ways of admitting frail elderly patients to the hospital can affect their care and recovery. Specifically, it looks at patients aged 75 and older who have a certain level of frailty (measured by a score) and who need hospitalization due to worsening health. The study aims to see if creating new care pathways, like having specialized teams or short-term hospital stays, can help improve outcomes for these patients and reduce their chances of needing to return to the hospital after discharge.

To join the study, participants must be at least 75 years old, be receiving care at a specific hospital in Toulon, and have a certain level of frailty. The trial will begin collecting data from the time patients are admitted and will continue for 90 days after their hospital stay. This research is important because it could help identify better ways to support frail elderly individuals, improve their healthcare experience, and optimize resources in the healthcare system.
