This clinical trial is studying a new approach to improve healthcare for residents in nursing homes, particularly those aged 70 and older. The goal is to see how effective a mobile team, made up of a geriatrician (a doctor specialized in older adults) and a nurse, can be in providing assessments and care directly in nursing homes. This team will help residents who face challenges such as managing multiple medications, frequent hospital visits, memory issues, and other medical complexities. The trial is set to take place in three nursing homes in Belgium and aims to establish a better standard of care for these residents.
To be eligible for this trial, participants need to be over 70 years old and have specific health concerns like difficulty planning for future healthcare, experiencing repeated falls, or having trouble with memory. However, those with unstable medical conditions that need urgent attention or are under 70 years old cannot join. If selected for the trial, participants can expect to receive personalized assessments and care from the mobile team, helping to improve their overall health and potentially reduce hospital visits. This pilot study will also gather valuable information to inform future larger studies on the cost and effectiveness of this approach.

Eligibility criteria
- Inclusion Criteria:
- • Patients aged over 70 years
- * Presenting with one or more of the following conditions:
- • Difficulties in establishing an advance care planning
- • Medical complexity (multiple specialists, repeated hospitalizations)
- • Polypharmacy
- • Recurrent falls
- • Unmanageable behavioral disorders
- • Weight loss
- • Memory impairment
- Exclusion Criteria:
- • Patients with an unstable medical condition requiring urgent care
- • Patients under 70 years old
- • Patients whose condition requires examinations or treatments available only in a hospital setting
