This clinical trial is looking at whether having a community paramedic visit patients at home can help manage heart failure better than the usual care they receive. Heart failure is a condition where the heart doesn't pump blood as well as it should. The goal is to see if these home visits can help patients avoid hospital stays, improve their health and quality of life, and make care more affordable. The study will also gather feedback from patients and doctors to see how well this approach works and if it can be used in the future.
To be eligible for this study, participants should be adults aged 65 to 74 who have been hospitalized for heart failure, need support at home, or have advanced heart failure that requires regular treatment adjustments. However, patients who need more intensive hospital care or have certain health issues that could affect their participation may not be included. If you join the study, you can expect close monitoring from a community paramedic who will help manage your heart failure at home. Overall, this trial aims to find a better way to help heart failure patients feel better and stay healthier.

Eligibility criteria
- Inclusion Criteria:
- One of the following criteria:
- • 1. Adults admitted to the hospital with decompensated HF eligible for early discharge with CP support and monitoring.
- • 2. Acutely decompensated HF patients at home or being evaluated in the ambulatory clinic who require large volume diuresis but do not require hospital-level monitoring.
- • 3. Stage D advanced HF patients who require frequent (\>weekly) diuretic adjustments (either inpatient or outpatients).
- Exclusion Criteria:
- • 1. Hospital-level monitoring or care is clinically indicated.
- • 2. Failed safety assessment, active substance abuse, or behavioral health diagnosis which could impact participation.
- • 3. Enrolled in hospice.
- • 4. Patient is a resident in a skilled nursing facility.
- • 5. Patient does not have a primary care physician or cardiologist at Mayo Clinic.
- • 6. Patient participants with communication barriers due to medical illness or cognitive impairment.
- • 7. Pregnant
