This clinical trial is studying how an intelligent intervention program can help improve frailty in older adults who are hospitalized, especially those with kidney disease. Frailty is a common condition among elderly patients, making them more vulnerable and dependent due to age and health issues. The goal of the study is to see if this specialized program, which lasts for 12 weeks, can reduce frailty, enhance daily living skills after discharge, and lower the chances of being readmitted to the hospital.
To participate in the trial, individuals must be 65 years or older, currently hospitalized in the nephrology department or undergoing dialysis, and able to communicate independently. They should expect to stay in the hospital for at least six days. Participants will be randomly assigned to either the group receiving the intelligent intervention or the group receiving standard care. The researchers will evaluate how well the program works by looking at changes in frailty levels and daily functioning 30 days after discharge. This study aims to provide useful strategies for healthcare providers to better care for frail elderly patients in the hospital.

Eligibility criteria
- Inclusion Criteria:
- • Hospitalized patients aged 65 years and older. (、
- • Admitted to the nephrology department or undergoing dialysis.
- • Conscious, able to express themselves independently, follow instructions, and communicate in Mandarin or Taiwanese.
- • Expected hospital stay of at least six days.
- • Assessed using the Physical Activity Readiness Questionnaire for Everyone (PAR-Q+) and deemed to have no immediate risk for exercise participation, with normal capability for physical activity.
- Exclusion Criteria:
- • Severe visual or hearing impairments that hinder communication or assessments.
- • Significant cognitive impairment, defined as a Montreal Cognitive Assessment (MoCA) score \< 24.
- • Patients who do not consent to participate in the study.
- • Individuals unable to personally complete the consent form.
- • Patients receiving palliative care.
- • Regular participants in rehabilitation therapy.
- • Charlson Comorbidity Index (CCI) score ≥ 5.
