ClinConnect Summary

This clinical trial is focused on helping older adults who have difficulty leaving their homes due to health issues. Researchers want to see if a special program using mobile health technology, called person-centred care (PCC), is more effective than standard care given by healthcare providers. The program involves six video call sessions through WhatsApp with nurse case managers over three months, where participants can receive support and guidance tailored to their needs.

To participate in this study, individuals need to be at least 60 years old, unable to leave home more than twice a week due to health problems, and have a smartphone they know how to use. It's important that they speak Cantonese and can understand the information being shared. However, those who are completely bed-bound, cannot communicate well, or live in areas without stable internet won't be eligible. This trial is not yet recruiting participants, but it aims to empower homebound older adults by providing them with personalized care and support through technology.
