This clinical trial is looking at two different treatments for patients with lumbar disc herniation (a condition where a disc in the lower back bulges or breaks). The goal is to see how well conventional physical therapy compares to a special technique called the flexion-distraction (Cox technique) in reducing pain, improving daily activities, and enhancing overall quality of life. Participants will be divided into two groups: one group will receive standard physical therapy using methods like ultrasound and electrical stimulation, while the other group will experience the Cox technique, which involves a gentle pulling motion on the spine. Both groups will also do some exercises at home.
To be eligible for this trial, participants need to be between 30 and 60 years old and have been diagnosed with a specific type of disc herniation. They should not have had any back surgery or serious health issues, and they need to be able to give their consent to participate. The study will be conducted at two local medical centers, and a total of 68 people will take part. Participants can expect to undergo treatment three times a week for four weeks, and their progress will be monitored by a trained physiotherapist. The results of this trial will help determine which treatment might be more effective for managing this painful condition without surgery.

Eligibility criteria
- Inclusion Criteria:
- • Being between 30-60 years of age
- • Having a diagnosis of L4-L5 lumbar disc herniation
- • Not having undergone surgical intervention
- • Having no cognitive impairment or intellectual disability
- • Providing voluntary participation and written consent
- Exclusion Criteria:
- • Having previously undergone surgical operation in the lumbar region
- • Having an oncological diagnosis
- • Pregnancy
- • Presence of severe physical or psychological disorders that would prevent completion of the study
- • Participation in another active physical therapy program
