This clinical trial is studying a new smart system designed to improve the quality of primary care for patients at community health service centers. The system uses advanced technology, including augmented reality (AR) glasses for doctors, to help them diagnose and treat patients better. Patients will also have access to a user-friendly interface that provides health education, reminders for follow-up care, and even virtual consultations using artificial intelligence. To participate in the trial, you need to be at least 18 years old and receiving care at one of the participating health centers. However, individuals with severe mental illness or those who cannot complete follow-up visits or questionnaires independently will not be eligible. - • Receiving care at a community health service center
- • Able to understand and comply with the study procedures
- • Agrees to participate in the study and signs the informed consent form
- Exclusion Criteria:
- • Patients with severe mental illness
- • Patients unable to independently complete the questionnaire
- • Patients expected to be unable to complete follow-ups within three months
