The GET HEALTH-I study is researching how a special remote care program can help older patients living in nursing homes after they have been discharged from the hospital. This study is currently looking for residents of any age who meet the criteria, but it does not include those who are only staying in the nursing home for a short time. By joining this study, participants can contribute to understanding how to better care for older adults after hospital stays.
