This clinical trial is looking at ways to improve depression treatment for older adults living with HIV in Nigeria. It compares two approaches: one group will receive the usual care for depression available at the HIV center, while the other group will get a stepped-care intervention. This stepped-care approach means that treatment will be tailored to each person's needs and will change based on how severe their symptoms are. The researchers want to see if this new method helps reduce depression and improve overall health outcomes related to HIV.
To be eligible for the trial, participants need to be at least 50 years old, living with HIV, and have a certain level of depression as measured by a simple questionnaire. If someone scores 10 or higher, they may qualify for the study. However, those at risk of suicide or with severe cognitive issues will not be included. Participants will be monitored over six months to see how effective the different treatments are, with assessments happening at regular intervals. It's important to note that this trial is not yet recruiting participants.

Eligibility criteria
- Inclusion Criteria:
- • Consenting individuals living with HIV
- • People living with HIV who are 50 years and above (PLHIV aged ≥50 years)
- • Those with a score of ≥10 on the 9-item patient-health questionnaire (PHQ-9).
- Exclusion Criteria:
- • • Older PLHIV will be assessed for the imminent risk of suicide and if there is an high risk, participant will be excluded.
- • Older PLHIV with severe cognitive impairment or diagnosed dementia that limits their ability to provide informed consent or complete study visits will be excluded.
- • Participants with comorbidities that can preclude the use of sertraline should be excluded.
