This clinical trial, called the Pre-MYO Cohort, is focused on understanding a heart condition known as myocarditis, which is inflammation of the heart muscle. The study aims to gather information from a large group of patients who may have this condition to learn more about their symptoms, treatment, and the factors that influence their health. Researchers will also test a new blood marker to see if it can help diagnose myocarditis more accurately than current methods. Additionally, the study will look into genetic factors and the different ways myocarditis affects men and women, especially considering age and other health aspects.

To participate in this trial, individuals should be experiencing symptoms that could indicate myocarditis, such as chest pain, shortness of breath, or unexplained heart issues, and have specific findings from heart tests. The trial is open to all eligible adults aged 65 to 74, and those who join will be contributing valuable information to help improve diagnosis and treatment for future patients. Participants will need to provide informed consent, meaning they agree to take part after understanding what the study involves.

Eligibility criteria

  • Inclusion Criteria:
  • • Presence of at least one compatible symptom and at least one objective finding in diagnostic tests (Figure 2), according to the consensus document of the European Society of Cardiology (1):
  • A. Clinical presentation:
  • Acute chest pain: pericardial or pseudo-ischemic.
  • Dyspnea on exertion or at rest for up to 3 months.
  • Dyspnea on exertion or at rest for 3 months - 1 year.
  • Unexplained palpitations / arrhythmias, sudden death recovered.
  • Unexplained cardiogenic shock.
  • B. Findings in complementary tests:
  • ECG: new onset AV block, new onset bundle branch block, ST segment elevation/depression, T wave inversion
  • Elevation of TnI/TnT
  • Segmental or global contractility alterations not explained by other causes
  • Positive Lake Louise criteria in cardiac MRI (13) (Annex III Figure S1) • Patients who have given their informed consent.
