ClinConnect Summary

This clinical trial is focused on improving care for people with Type 2 Diabetes who come to the emergency department (ED) with high blood sugar levels. Researchers are testing a system that uses electronic alerts in the hospital's record-keeping system to help doctors provide faster and better treatment. When a patient's blood sugar is very high (250 mg/dL or more), an alert will remind the doctor to check another important blood test called hemoglobin A1c (A1c). If the A1c is also high (10% or more), the alert will suggest that the patient may need additional care support. The goal is to see if these alerts lead to quicker treatment and better diabetes management.

To participate in this study, patients should be adults aged 70 or older who arrive at the ED with high blood sugar levels but are not pregnant or have recently tested positive for COVID-19. Patients with serious diabetes complications or those who decide to leave the ED against medical advice won’t be eligible. Those who join the trial can expect to receive regular care while also being part of this important study to help improve diabetes treatment in emergency situations.

Eligibility criteria

  • Inclusion Criteria:
  • Moderate hyperglycemia, (glucose ≥250 mg/dL)
  • Patients who arrive in the emergency department
  • Not pregnant or peri-partum
  • Not SARS-COV-2 PCR positive in past 7 days
  • Exclusion Criteria:
  • Diabetic ketoacidosis (pH \< 7.20, HCO3 \< 15, AG \> 25)
  • Diabetic foot ulcer or skin complications
  • Hyperglycemic hyperosmolar state with neurologic impairment
  • Patients who leave against medical advice (AMA), elope from the ED, or are transferred to another facility
