This clinical trial is studying ways to prevent breathing problems that can occur after surgery, especially for patients who are under general anesthesia for a long time. When patients are put to sleep for surgery, their lungs can sometimes struggle to expand properly, which can lead to serious issues after the operation. The researchers want to find out if using advanced monitoring techniques during a specific lung procedure (called alveolar recruitment maneuvers) can help keep the lungs open better after surgery. They will compare traditional methods to see if the new monitoring provides better information about how to support the lungs.
To be eligible for this trial, participants must be at least 18 years old and scheduled for a surgery that lasts more than two hours, particularly if they are obese or have certain surgery positions. Participants will have their lung function monitored with advanced technology during their surgery, and they will be informed about the study and give their consent to participate. It's important to note that this study is not yet recruiting participants, but it aims to improve patient care by ensuring better lung health after surgery.

Eligibility criteria
- Inclusion Criteria:
- • 1. Patients ≥ 18 years of age
- 2. undergoing :
- • scheduled surgery at Hospital Lariboisière.
- • scheduled to last more than 2 hours under general anesthesia for abdominal surgery under laparoscopy or surgery in the prone position, or for obese patients with a BMI \>35.
- • And for whom the ventilator used includes advanced monitoring
- • for whom placement of a gastric tube is indicated either because of the duration of the procedure or because it is necessary for the surgery.
- • 3. Patient informed and expressing non-opposition to participation in this study
- Exclusion Criteria:
- • - Patients under 18 years of age.
- • Contraindications to esophageal tube placement
- • Contraindications to ARMs
- • Patient opposed to protocol participation
- • Pregnant, parturient or breast-feeding women
- • Patient under legal protection
