This clinical trial is studying the effects of two different doses of Continuous Renal Replacement Therapy (CRRT) in critically ill patients who have acute kidney injury. CRRT is a treatment used in intensive care units (ICUs) to help patients whose kidneys are not working properly. It works continuously to remove toxins and extra fluid from the body, which is important for recovery. The trial aims to find out how different doses of CRRT affect the balance of important substances in the body, like sodium and potassium, as well as the loss of vital nutrients during treatment.
To participate in the trial, patients need to be at least 18 years old and have severe acute kidney failure that requires CRRT. They must also be receiving certain medications to support their blood pressure. During the study, patients will be randomly assigned to receive one of the two doses of CRRT for 96 hours. Their doctors will monitor them closely to ensure their safety and adjust treatment as needed. After the treatment period, there will be follow-up to check on their recovery about 90 days later. The risks involved are minimal, as patients will be monitored more carefully than usual. The study plans to include around 100 participants.

Eligibility criteria
- Inclusion Criteria:
- • Patients 18 years of age or older
- • Acute renal failure (AKIN 3) requiring Continuous Renal Replacement Therapy
- • Requirement of vasoactive drugs (norepinephrine \> 0.1 mcg/kg/min)
- • Written informed consent (patient or family)
- Exclusion Criteria:
- • Lack of informed consent.
- • Possibility of using plasma exchange or MARS therapy at the at the time of inclusion.
- • Baseline renal function with an estimated glomerular filtration rate (CKD-EPI) less than 30 ml/min.
