This clinical trial is studying how better teamwork between hospital pharmacists and community pharmacists can help patients who are going home after being hospitalized. When patients take many medications, their prescriptions can change during their hospital stay, which sometimes leads to mistakes when they go back to their regular pharmacy. The researchers want to find out if having a hospital pharmacist communicate important information directly with the community pharmacy can reduce these medication errors for adult patients who are taking seven or more medications.
To be eligible for the trial, participants should be adults aged 65 to 74 who have spent more than 48 hours in the internal medicine ward and are going home with seven or more prescribed drugs. They also need to be able to give consent, meaning they understand the study and agree to participate. If someone joins the study, their hospital will send necessary information to their usual pharmacy, but they won’t have to do anything extra besides visiting that pharmacy after discharge. The trial is not yet recruiting participants, but it aims to make the process of managing medications smoother and safer for patients after they leave the hospital.

Eligibility criteria
- Inclusion Criteria:
- • Patients admitted to the internal medicine ward for more than 48 hours
- • Patients prescribed seven or more drugs at the time of screening
- • Patients discharged to home
- • Patients able to give informed consent as documented by signature
- Exclusion Criteria:
- • Patient discharged to another hospital, nursing home or rehabilitation clinic
- • Refusal of the community pharmacy to participate
- • Inability to sign consent and follow the procedures of the study, due to language problems, psychological disorders, dementia, alterations of consciousness and lack of judgement
