ClinConnect Summary

This clinical trial is looking at how two simple tests, the Sit to Stand Test and the Gait Speed Test, can help physiotherapists make better decisions about when older adults can safely leave the hospital after being admitted for an acute illness. The goal is to ensure that older patients, aged 65 and older, are ready to go home in a safe and timely manner, helping them recover better once they leave the hospital.

To join this study, participants must be 65 years or older, able to walk at least 5 meters on their own or with a walking aid, and have a certain level of mental ability to follow instructions. However, those who have severe physical mobility issues or certain mental challenges won't be eligible. If someone decides to participate, they will undergo these tests to help evaluate their readiness for discharge, and the results will contribute to improving discharge planning for older adults in the future.

Eligibility criteria

  • Inclusion Criteria:
  • Aged 65 years and older
  • Able to ambulate without physical assistance with or without walking aids as per premorbid status for at least 5 metres
  • Clinical Frailty Scale (CFS) of ≤ 5
  • Able to follow instructions (Abbreviated Mental Test (AMT) ≥ 5)
  • Exclusion Criteria:
  • Admitted for acute orthopaedic or neurological conditions with physical deficits that affect functional mobility
  • Requires physical assistance for functional mobility from ≥ 1 person
  • Clinical Frailty Scale (CFS) of \> 5
  • AMT \< 5 (Lack capacity to consent to study)
