This clinical trial aims to explore how virtual health coaching can help people in underserved communities who may not have enough food and are at risk for heart disease and related conditions like high blood pressure, high cholesterol, and diabetes. The main goals are to see if personalized coaching can lower the chances of having a heart attack or stroke over the next ten years, reduce the rates of these health issues, and improve access to healthcare. Participants will take part in health screenings and virtual sessions where they can discuss important topics such as healthy eating, exercise, and managing their health.
To be eligible, participants need to be between 40 and 75 years old, either speak English or Spanish, and have at least one health risk factor, like high blood pressure or obesity. The study will not accept those who are younger than 40 or older than 75, are pregnant, or do not have a way to connect to virtual sessions. If you join the study, you can expect to attend health screenings in person and have regular online coaching sessions to help you make healthier lifestyle choices. The trial is not yet recruiting, but it offers a chance for individuals to take an active role in improving their health in a supportive environment.

Eligibility criteria
- Inclusion:
- • Patient is a participant at a CHI sponsored, community screening event Patient provides informed consent and willingness to participate for the duration of the study English or Spanish speaking Age 40-75 years old At least 1 cardiometabolic risk factor at suboptimal level
- Defined as one or more of the following at initial screening:
- Blood Pressure (BP):
- • Systolic BP greater than 130mmHg and/or diastolic BP greater than 80mmHg. Obesity: body mass index (BMI) greater than 30 〖kg/m〗\^2
- Dyslipidemia:
- • Total cholesterol greater than 200 mg/dL Triglycerides greater than 150 mg/dL Low density lipoprotein (LDL) greater than 130 mg/dL High density lipoprotein (HDL) less than 40 mg/dL in men or less than 50 mg/dL in women Hemoglobin A1c greater than or equal to 5.7% 10-year ASCVD score greater than 5% Access to a telehealth compatible device
- Exclusion:
- • Adults unable to provide informed consent or unwilling to participate for study duration Speaks language other than English or Spanish Younger than 40 years old or older than 75 years old All cardiometabolic risk factors at optimal levels No telehealth compatible device Pregnant patients
