This clinical trial, called RAPS-ICU, is focused on adapting a special assessment tool designed to help identify the risk of pressure injuries (also known as bedsores) for patients in Intensive Care Units (ICUs) into Turkish. The goal is to ensure that this tool works well in a Turkish-speaking context by testing its reliability and how well it measures what it is supposed to measure. This will help healthcare providers better assess and prevent pressure injuries in ICU patients.
To participate in the study, individuals must be 18 years or older, have been in the ICU for at least one day, and be willing to join the study either themselves or through their family members. Participants should be fully aware of their surroundings and not have any psychiatric issues that affect their ability to communicate. This trial is currently not recruiting participants, but once it starts, those who join can expect to help improve the care of future patients in ICUs by contributing to important research.

- Inclusion Criteria:
- • 18 years of age or older,
- • Staying in the Intensive Care Unit for at least 1 day,
- • Agreeing to participate in the study by themselves or a family member.
- • Volunteering to participate in the research
- • Full orientation and no psychiatric problems
- Exclusion Criteria:
- • - Having communication problems
