ClinConnect Summary
The PALMA project is a clinical trial aimed at improving care for older adults who are transitioning from the emergency department to primary care. It focuses on better understanding and managing palliative symptoms—those uncomfortable feelings or issues that arise when someone is facing a serious illness. The goal is to create a more organized way to assess these symptoms in elderly patients, helping to ensure clear communication between hospitals and community health services. This study will also look at factors like frailty (how fragile or weak someone may be) and social risks that could affect their care.
To be eligible for the study, participants must be older adults, specifically those aged 65 and above, who are admitted to the emergency department. However, individuals with severe cognitive impairment (difficulty thinking clearly) or those who cannot read Danish will not be included. If someone participates in this trial, they can expect to take part in assessments that help healthcare providers understand their needs better and improve their overall care experience. The PALMA project is currently not recruiting participants but is an important step toward enhancing palliative care for older patients.
