This clinical trial is studying a new way to help patients with chronic diseases receive better care. UCLA Health has a program called Proactive Care that aims to provide personalized support to patients who may need it most. Starting in December 2024, they will use an AI (artificial intelligence) tool to identify patients who are predicted to have high healthcare costs in the coming year. Each month, the program will select the top 250 patients identified by this tool to enroll in Proactive Care, while the remaining patients will be placed on a waitlist for 10 months unless their doctors request earlier enrollment.
To be eligible for this trial, participants must be at least 18 years old, enrolled in a UCLA Managed Care Plan, and identified by the cost prediction model as likely to have high healthcare costs over the next year. However, individuals who are already enrolled in any UCLA care management program or who have been in one in the past year cannot participate. If you join the trial, you can expect to be contacted by a nurse if selected for enrollment, and you will receive tailored support to help manage your health needs.

- Inclusion Criteria:
- • At least 18 years of age
- • Enrolled in a UCLA Managed Care Plan
- • Cost prediction model identifies patient as having high predicted costs over the next 12 months
- Exclusion Criteria:
- • Currently enrolled in any UCLA care management program
- • Enrolled in any UCLA care management program in the last 12 months
- • Already has an active referral to a care management program
