This clinical trial is studying how a serious condition called traumatic hemorrhagic shock (THS) affects blood flow in the kidneys, which can lead to a problem known as acute kidney injury (AKI). AKI is a complication that can make recovery more difficult after a serious injury, and understanding how blood flow changes in the kidneys during THS could help improve patient care. The researchers want to learn more about these changes to better manage and treat patients who are at risk of AKI after trauma.
To participate in this trial, you need to be over 18 years old and admitted to the intensive care unit (ICU) within 24 hours of a traumatic injury. You should also have received blood products during your initial treatment and have a certain level of a substance called lactate in your blood. However, if you are pregnant, have severe kidney failure, or have injuries that are not survivable, you won't be eligible for this study. If you join, you can expect to undergo tests that involve specialized ultrasound to help the researchers gather information about your kidney blood flow. This study is currently looking for participants, and your involvement could contribute to important findings that help others in the future.

Eligibility criteria
- Inclusion Criteria:
- • Age \> 18 years
- • Within 24 hours of ICU admission following traumatic injury
- • Received any blood products during initial resuscitation
- • Lactate \> 2 mmol/l at any stage prior to study enrolment
- Exclusion Criteria:
- • Known intolerance to Sonovue or any other ultrasound contrast agent
- • Patients with un-survivable injuries / not expected to survive 24 hours in whom the intent of treatment is palliative
- • Known CKD 4 or end stage renal failure
- • Pregnancy
